BUD RANK KINDERGARTEN REGISTRATION INFORMATIONAL FLYER

KINDERGARTEN 2010-11 PACKETS WILL BE AVAILABLE TO PICK UP STARTING WEDNESDAY, MARCH 3 @ 2:00PM IN THE BUD RANK OFFICE

Packets are numbered 1-100; the first 80 completed registrations are guaranteed a spot in BR Kindergarten.  Address will be checked at packet pick-up to confirm resident status.

2010-11 KINDERGARTEN REGISTRATION BEGINS MARCH 10

Your child must turn five years old by December 2, 2010 in order to meet the age requirement for kindergarten in the 2010-11 school year. If you have any questions, please call the Bud Rank office at 327-4900.

 

Registration for the CURRENT 2009-10 school year:

Bud Rank registration for the 2009-10 school year can be done anytime during office hours at the Bud Rank office.  The following items will need to be completed and brought in at the time of registration:
 

1. Your child’s birth certificate
2. Your child’s social security card or tax form with number on it
3. Your proof of residency (PGE bill, rental / lease agreement or escrow papers)

4. Your child’s current immunization record  (see below for immunization information)
    4 Polio (IPV/OPV) immunizations
    5 DTP shots
    3 Hepatitis B shots
    2 MMR (Measles, Mumps, Rubella) shots
    1 Varicella (chicken pox) shot
    1 TB skin test (read any time AFTER August 2009 for Kindergarten)

CURRENT 2009-10 REGISTRATION FOR GRADES 1ST-6TH

CHILD IS CURRENTLY IN CLOVIS UNIFIED SCHOOL DISTRICT

* NOTE! IF YOU CHILD IS ALREADY AT BUD RANK, YOU DO NOT HAVE TO RE-REGISTER

You may come to the Bud Rank Office with the following information:
 

1. Your child’s current school & grade information
2. Your proof of residency (PG&E bill, rental/lease agreement or escrow papers)


IF CHILD IS CURRENTLY IN ANOTHER SCHOOL DISTRICT

You may come to the Bud Rank Office and pick up a registration packet.

After completing the information in the packet return it to the office along with the following:
 

1. Your child’s birth certificate
2. Your child’s social security card or tax form with number on it
3. Your proof of residency (PG&E bill, rental/lease agreement or escrow papers)
4. Your child’s current immunization record

NURSE’S NOTE!!

All incoming first graders MUST have a physical prior to beginning first grade.  If your child had a physical during their kindergarten year between March 1, 2009 and now, that will meet this requirement.  If your child has not then he/she will need to have a physical in order to meet the state mandate.  The Sierra Vista Clinic has moved to the Tarpey portable just North of the kindergarten classrooms. The number is 327-8096.  The Fancher Creek Clinic’s phone number is 327-6742.  Appts are needed for physicals.  The clinic also provides immunizations. Please take advantage of this service.

Immunization Information

Fresno County requires that each child entering kindergarten receive the Mantoux intradermal skin test within one year of entrance into school (includes preschool). The skin test must be read and documented by a physician or health care professional trained in determining positive or negative readings. The Hepatitis B series (3 shots) are also required for children entering kindergarten. Effective July 1, 2001, all entering kindergarten children are required to receive the Chickenpox vaccine (varicella) or have documented proof (by a healthcare professional) that they have had the disease. Any student registering in California schools from out of state, grades 1-12, without documentation of having had Chickenpox, will also need the varicella vaccine. Registrations will not be completed without proof of the above immunizations. For your convenience, Clovis Unified School District, in cooperation with the State Department of Health and the Fresno County Health Services Agency offers free immunizations during the school year if your child is not up-to-date.
